A 2-year-old intact male DSH was presented for evaluation of anorexia. Abnormalities on CBC and serum biochemistry were anemia, hypoalbuminemia, and elevated AST and ALP activity and bilirubin.

Renal lymphoma vs FIP

Sonographic Differential Diagnosis

Renal lymphoma pattern. Possible FIP.
Aggressive medullary rim sign and bilateral renomegaly was noted. Regional lymphadenopathy. Potential early infiltrative hepatic disease.
Lymphoma or FIP are the primary differentials.

Image Interpretation

The right kidney was severely enlarged and measured 5.9 cm with medullary rim sign and disruption of architecture. Ill-defined pelvic fat was noted. This is strongly suggestive for infiltrative disease such as FIP or lymphoma. The left kidney presented similar changes to the right with more severe enlargement and measured 5.9 cm with medullary rim sign. Significant inflammatory pattern was noted around both kidneys. FNAs of the kidneys were performed.
The liver was mildly hypoechoic. Free fluid was noted between the liver lobes. The common bile duct measured 0.19 cm. FNAs of the liver were performed.
