A 6-year-old NM Labrador/Rottweiler mix was presented for evaluation of intermittent chronic vomiting, weight loss, and decreased appetite. He had been previously been diagnosed with vitamin B12 deficiency and had responded to supplementation and a hypoallergenic diet trial. Urinalysis showed mild proteinuria and hematuria. Abnormalities on laboratory testing were subnormal B12, elevated folate, and normal TLI.

DX

Jejunal FB

Sonographic Differential Diagnosis

Distal small intestinal obstruction.
Likely concurrent underlying inflammatory disease.
Hard linear foreign body.
Recommend immediate exploratory surgery with GI biopsies given the opportunity to assess for underlying chronic disease.

Image Interpretation

Gastrointestinal tract revealed a distal small intestinal obstruction with dilated loops of bowel followed by empty bowel and a hard shadowing foreign body in mid abdomen likely jejunum measuring approximately 3 to 4 cm. Retention of ingesta noted. Slight amount of free fluid noted.
