 A 7-year-old NM Australian shepherd dog was presented for evaluation of hematochezia that responds to symptomatic therapy but recurs when off medication.

DX

Cytology revealed colonic carcinoma.

Sonographic Differential Diagnosis

Colonic stricture at the level of the cystourethral junction.
If the sonographer is comfortable with ultrasound-guided FNA of this lesion, this could be performed. Primary differentials include granulomatous lesion, colonic carcinoma especially given the focal mineralization and colonic lymphoma. Alternatively, surgical intervention with subtotal colectomy would be recommended. However, the surgeon may find difficulty in reaching the distal portion for resection and anastomosis given the position. Guarded prognosis depending upon underlying histopathology.

Image Interpretation

The colon in this patient presented a focal thickening with a structuring contour that measured 2.04 x 0.96 cm with a focal area of mineralization in the deep descending colon. The stricture appears to be at the level of the trigone and cystourethral junction. The upper gastrointestinal tract was unremarkable.

Outcome

Follow-up ultrasound revealed no evidence of metastasis and normal resection and anastomosis of the descending colon was noted.

Cytology revealed colonic carcinoma.
