A 17-year-old SF DSH was presented for evaluation of recent onset vomiting, inappetence, and lethargy. Abnormalities on CBC and serum biochemistry were mild anemia, elevated  amylase and ALP activity, and severely elevated ALT activity (874).

DX

Proximal descending colonic mass.

Sonographic Differential Diagnosis

Proximal descending colonic mass. This appears resectable. The primary differentials include colonic lymphoma, colonic carcinoma or mild potential for complicated inflammatory disease. Chest radiographs prior to surgery would be recommended to ensure that metastatic disease is not an issue.

Image Interpretation

The descending colon in this patient revealed an infiltrative obstructive mass. The proximal segment measured 2.35 cm lying in the left cranial abdomen. In the mid to distal descending colon, a segment measuring 5.85 cm was also identified. This appears to be resectable with subtotal colectomy. The infiltrative descending colonic pattern appears to enter close to the pelvic inlet. Removal of the entire colon to the level of the cystourethral junction would be recommended.
