03_00280 Teddy S Cholecystitis of gallbladder remnant BK

An 8-year-old NM Sheltie dog with a history of elevated liver enzyme activity and previous cholecystectomy was presented for evaluation. Abnormalities on serum biochemistry were elevated liver enzyme activity.

Sonographic Differential Diagnosis

Post surgical dilation of the residual cystic duct. Secondary local inflammation. The changes reveal post surgical dilation of the residual cystic duct and secondary local cholecystitis. The patient should be treated with Baytril, Metronidazole and Actigall.

Image Interpretation

The liver was of normal echogenicity and was hypoechoic to the spleen and isoechoic to the kidneys. A gallbladder was not visualized due to the cholecystectomy. The residual cystic duct appeared dilated and the adjacent hyperechoic tissue was likely secondary to surgery.

DX

Samples of the gallbladder remnant reveal chronic active fibrosing cholecystitis and mild chronic lymphoplasmacytic cholangiohepatitis.

Outcome

The patient was asymptomatic 2 weeks post surgery. Two months later a gallbladder mucocele was noted.

Sampling

Samples of the gallbladder remnant reveal chronic, active, fibrosing cholecystitis and mild chronic lymphoplasmacytic cholangiohepatitis.
