An 11-year-old MN DLH was presented for potential urethral obstruction as he was showing stranguria and hematuria with a history of prior urethral obstruction and idiopathic cystitis. Current diet is c/d, which according to the owner is causing diarrhea.

DX

Histopathology of the liver revealed cystadenoma.

Sonographic Differential Diagnosis

Left-sided liver mass. This appears resectable with left lobectomy. Interstitial cystitis bladder pattern. Potential bladder lymphoma or transitional cell carcinoma. Sampling of the bladder is essential in this case. Surgical biopsy would be best at the time of left lobectomy. Alternatively, traumatic catheterization of the bladder wall would be recommended with ultrasound-guidance.

Image Interpretation

A large, cystic mass was noted in the cranial abdomen in this patient and measured 5.32 x 3.49 cm. It appears to be deriving from the left liver with micro and macrocystic changes. This is most consistent with cystadenoma or potential biliary adenocarcinoma. It appears resectable with full left sided lobectomy as plenty of adequate liver appeared present. The portal vein was impinged upon by the mass; however, the vena cava did not appear to be affected. The gallbladder was not visualized. Further intercostal approaches to the portal hilus would be ideal in this case.

Outcome

The surgeon was unable to fully resect the mass. No further outcome provided.
