A 13-year-old NM canine mix was presented for evaluation of vomiting and anorexia, Abnormalities on CBC and serum biochemistry were left shift neutrophilia, elevated liver enzyme activity, hyperglobulinemia, azotemia, and hypercholesterolemia. In addition there was abdominal pain with imaging.

Sonographic Differential Diagnosis

The gallbladder had aspects of mucocele and chronic cholecystitis. Therefore, long term function of the gallbladder is likely going to be minimal and will continue to be a problem. Cholecystectomy and common bile duct lavage would be warranted. IV fluid therapy, broad spectrum antibiotics and pain management would all be indicated. Medical treatment could be attempted, but is considered temporary stabilization.

Image Interpretation

The liver presented increased portal markings and remodeling. Dilated gallbladder was noted with suspended debris and echogenic wall. The proximal common bile duct was also dilated. The gallbladder measured 9 x 5 cm with significant dilation of the neck and echogenic debris around the region of the neck, which suggests inflammation.

The patient passed away.
