03_00198 Madison B Hepatic cyst

A 12-year-old MN Persian was presented for Rabies vaccination and to clip the fur on the hindquarters. On physically examination body score condition was 2ВЅ/5, a grade III/VI systolic murmur was present, the abdomen was bloated, distended and painful and there was a mass lesion on the right cranial abdomen adjacent to the body wall. CBC, serum biochemistry, and T4 were all within normal limits. Urinalysis was performed.

Sonographic Differential Diagnosis

Large, left sided, hepatic cysts. Concurrent gastritis and gastric deviation from the left liver cyst.

Image Interpretation

The abdomen in this patient presented a large, mildly anechoic cyst with echogenic debris that occupied most of the left liver and deviated the stomach caudally. This appears resectable with full left lobectomy. The stomach presented multifocal, hypoechoic changes in the gastric wall.

DX

Large, left sided, hepatic cysts. Polycystic kidney disease. Gastritis and gastric deviation.

Outcome

Surgery was scheduled to be performed, but no further outcome was available.

Comments

The patient was lost to follow up.

Clinical Differential Diagnosis

Cardiac disease – dilated/hypertrophic cardiomyopathy, tricuspid valve insufficiency, myocarditis. Abdominal mass – hydronephrosis, renal/splenic/lymph node neoplasia/granuloma, cyst. GI tract – partial obstruction, neoplasia.

Sampling

Surgery was scheduled to be performed, but no further outcome was available.
