The patient is a canine Jack Russell, SF, 11 years old who was presented for vomiting, diahrrea and PU/PD. The patient was painful on abdominal palpation. CBC, Chem and UA yieled the following results: AST 228, ALT 536, ALKP 1051, white count 27000 with neutrophilia. Urine specific gravity 1.019, 3+ protein, pH 6.5.
Left sided liver mass, pedunculated. Suspect potential torsion. Significant inflammation was noted. Mild left liver nodule, likely an extension of the same pathological process. Hepatocellular carcinoma or similar is suspected. The mass appears particularly aggressive, yet at this time it appears amenable to left lobectomy. Emergency surgery is recommended. No evidence of metastatic disease was noted. The spleen appears unremarkable. Chest radiographs are recommended. Guarded prognosis.
The liver in this patient presented a left sided, complex, cystic mass deriving from the caudal aspect of the left lateral liver lobe with peri-mass inflammatory pattern. A second target type nodule was noted in the deep left liver with capsular expansion. A minor amount of free fluid was noted. The deep right liver appeared structurally unremarkable. Regional inflammation was present. The gallbladder was unremarkable with a minor amount of debris. Lobe torsion is significantly possible in this case.
