Finding adrenal glands has always been a challenge but I was hoping a confirmed Cushing’s case would be less of a challenge. These images are from a 13 year FS beagle with a history of PU, PD, and PPP. Bloodwork- elevated ALP, ACTh stim consistent with Cushing’s.
1) Is the first image consistent with an enlarged adrenal gland? I was unable to get the characteristic peanut shape but used the aorta and left kidney as my landmarks.

2) The right adrenal was definitely a serious challenge especially since I was dealing with so much gas from the intestines.

I actually watched your YouTube video several times and found it very helpful( especially ” reducing the real estate”). Can you see the 1st image with what I found to be in the location of the left adrenal and the aorta? What is your opinion of the image?

The first image you have
The first image you have small intestine in the near field. You want to have body wall, fat and aorta/left renal artery in your field and nothing else. Any other structure in the near field (2-3 cm), whether small or large intestine, will steal your echos and cause for poor image quality in the far field 3-4 cm which is where you want the AO/LRA to target the left adrenal just cranial to that point of reference. The kidney is pushed to the left out of view with proper widespread manual pressure.

I think you may have a piecve
I think you may have a piecve of it but tough to tell on a still. Can you upload a video? That way we can see the AO and Left renal artery to be sure this is not a LN or something similar.
As a rule I have learned to never make any significant evaluation on a still image becaus eyou can make pathology out of a normal structure or miss th epathology altogether or in this case call a LN or pancreatic lesion an adrenal…
