cyst vs biliary cystadenoma appearance?

Hello there,

Bella is a 14 year old DSH F/S that presented with vomiting. Bloodwork NAF, US revealed some minor GB sludge, distended CBD 3.3-3.5 mm. Rest of the abdomen was normal. The cat was placed on Ursodiol and Ab’s and improved with no more vomiting and back to normal.  After 2 weeks at recheck the CBD looks the same but now there is a “cyst like ” lesion on the left medial liver lobe. I’m pretty sure this lesion was not present during the previous US exam.

Q: can this be an intrahepatic bile duct cyst due to new meds( ursodiol) or is this more likelly Biliary cystadenoma which might be also the reason for the mild CBD distension?

CBDs get bigger in older cats

CBDs get bigger in older cats like the panc duct and if still < 0.4 likely not an issue if no bili elevation either or SAP rise. The cystic lesion would be typical old cat cystadenoma but biliary adenocarcionoma can do this but will grow and be more disorganized so just watch and remove if it grows or becomes pedunculated to predispose to lobe torsion.
