DX

Chronic hydronephrosis with strictured left ureter and hydroureter with small intestinal entrapment and regional inflammation/adhesions.

Image Interpretation

The left kidney is severely hydronephrotic with generalized enlargement measuring 6.94 cm. Hydronephrosis measured 3.75 x 6.23 cm. Minimal residual cortical parenchyma was present at the caudal cortex. The left ureter in this patient was dilated up to 1.4 cm and strictured in a position of approximately 4.0 cm caudal to the left kidney. Concurrent small intestinal entrapment was noted with regional inflammation and adhesions.

Outcome

Exploratory surgery to free up the adhesions was recommended. IVP to assess if viability of the left kidney is still present in order to save the left kidney, would be appropriate. However, direct left nephrectomy, liberation and left ureterectomy may be appropriate in this patient. IVP would be the best way to assess which treatment is more viable either left nephrectomy or decompression surgery by freeing the left ureter.
The right kidney appears completely viable and normal. There was no evidence of neoplasia
