The kidneys presented moderate chronic interstitial nephrosis pattern. Dystrophic mineralization and slight pyelectasia was noted in the left kidney. Cortical collapse was noted at the caudal pole of the right kidney. A left adrenal gland mass was noted in this patient. This measured 2.6 cm with no evident vascular invasion noted. Expansion into the phrenic vein was noted in this patient to approximately 1.0 cm; however, the vena cava was not obviously involved.
Stabilization of the kidneys would be recommended with staging towards left
adrenalectomy. The kidneys appear moderately compromised; however, the adrenal
pathology may be driving renal failure in this patient. Therefore, stabilization of the
kidneys and attempt at left adrenalectomy with treatment for chronic renal disease
would be recommended as the next approach assuming that no metastatic patterns are
noted in the chest.
