RAD – Generalized hepatomegaly with pot belly and bladder stone suggestive for Cushings in a 10 year old M Toy Poodle

This 10 year old M toy poodle dog presented for vomiting, lethargy and inappetance which resolved with supportive treatment in 2 days. Bladder stone noted on radiographs. Painfulin cranial abdomen.

Image Interpretation

right lateral and VD abdomen – The abdominal wall is pendoulous. A moderate amount of intra- and extraabdominal fat is noted. The serosal detail is within normal limits. The urinary bladder is moderatly distended, one stone of 1.5 cm diameter is seen within the urinary bladder. The stomach is contains a mild amount of food and gas. The gastric axis is rotated caudally owing to the hepatomegaly.  The liver reveals moderate generalized enlargement with rounded lobar margins.

DX

• Large non-obstructive urinary bladder stone • Moderate generalized hepatomegaly • Pot-bellied abdomen • Obesity • No evidence of obstructive gatrointestinal pathology

Outcome

Consider diffuse infiltrative hepatopathy such as vacuolar, steroid induced, inflammatory and neoplastic.
With the sum of the findings a systemic endocrinologic syndrome such as Cushing’s, Diabetes or other should definetely be conisdered. Clinical and laboratory correlation is warranted. Also consider a full abdominal ultrasound with sampling of the liver parenchyma for further definition
