An 11-year-old spayed female DSH cat was presented for evaluation of anorexia with survey radiographs showing ascites. Abnormalities on blood work included elevated BUN, ALP, and and lipase.
Abdominal neoplasia involving pancreas and likely metastatic disease to the liver with nodular omental changes. Carcinomatosis or similar neoplasia suspected.
Pancreas was hypoechoic, nodular, and irregular. This is likely the site of primary neoplasia. A moderate amount of ascites was noted. There were nodular omental changes. The liver presented multiple mixed echogenic masses occupying most of the parenchyma; these were non-resectable in the deep left, right and caudal aspects of the liver. There was a swollen irregular contour and pericapsular inflammatory pattern. The gallbladder was unremarkable.

Ascites: transudate, cardiac disease, neoplasia, septic peritonitis, non-septic peritonitis (bile, urine, FIP), hemorrhage; Chronic kidney disease.
Ultrasound-guided fine needle aspirate of the intestine was consistent with a carcinoma. The FNA also contained a rare bacteria which was most likely a contaminant.
