Peripancreatic sponge foreign body in a 3 year old MN Pointer **April 2013 COM**

A 3-year-old neutered male Pointer cross dog was presented for vomiting/diarrhea. Endoscopy of the stomach and duodenum was within normal limits. Blood work was unremarkable, but palpation of the cranial abdomen revealed some discomfort.

DX

Peripancreatic abscess, foreign body

Sonographic Differential Diagnosis

There is a large pancreatic abscess with peri-pancreatic inflammation and gastrointestinal impingement. Spasmic duodenum and duodenitis is also present. There is the possibility for foreign body embedded within this abscess given the organization. At times, foreign bodies can pass through the intestinal wall and embed into the abscess.

Image Interpretation

The stomach tract presented relatively normal mucosa with an empty lumen. The axis of the pyloric antrum was deviated by a large progressively shadowing pancreatic mass, abscess, and or necrosis occupying the right pancreatic base (Images 2-4 and Video 2-4). It measured approximately 4.0 cm with shadowing material within the lumen. Peri-pancreatic inflammation and acute on chronic mixed echogenic tissue changes were also noted associated with the pancreas. Hi resolution linear image demonstrates spasming of the proximal duodenum adjacent to the lesion with periserosal and peripancreatic ill-defined echogenic fat suggestive for inflammation (Video 5).

Outcome

A gauze sponge was removed surgically without complication. Debridement of the necrotic tissue was performed. The patient had undergone gastrointestinal surgery in the past at another facility, but the history was unclear. The patient recovered uneventfully.

Clinical Differential Diagnosis

Vomiting/diarrhea – non-specific gastro-enteritis (viral, bacterial, helminths, protozoa, dietary indiscretion, toxins), dietary hypersensitivity, IBD, foreign body, emerging lymphoma, pancreatic pathology (pancreatitis, abscessation),
Addison’s disease.

Sampling

An exploratory laparotomy was conducted.
