Prostatitis, paraprostatic cyst, and testicular neoplasia in a 10 year old MI Labrador Retriever dog

A ten year old intact male labrador retriever was presented with an enlarged prostate and painful rear end. Serum biochemistry and CBC were within normal limits. The urine specific gravity was 1.022 with mild pyuria.

Sonographic Differential Diagnosis

Testicular mass and prostatitis with paraprostatic cyst. There is a minor potential for prostatic neoplasia. Neutering and FNA of the prostate with culture of the drained fluid would all be warranted with a recheck sonogram in 3 weeks.

Image Interpretation

The patient revealed a hypoechoic mass on the right testicle that measured 1.4 x 1.43 cm. The prostate was enlarged with swollen contour and microcystic changes. Minor subcapsular fluid accumulation was noted. The prostate measured 5.58 x 5.18 cm. A cyst deriving from the cranial aspect of the prostate was noted; this is consistent with paraprostatic cyst or abscess that measured 13 x 5.7 cm.

Testicular mass, prostatitis with paraprostatic cyst.

Outcome

No further outcome at this time.

Comments

Culture of the aspirated fluid would be recommended; aggressive antibiotic therapy such as a combination of Enrofloxacin and Clavamox would be warranted.

Sampling

U/S guided FNA of prostate. 135cc of fluid was drained from the paraprostatic cyst. Cytology of the fluid revealed mild epithelial cell hyperplasia with iatrogenic hemorrhage.
