A 12-year-old NM Boston terrier that had been previously diagnosed with congestive heart failure 6 months ago was presented for follow up evaluation. Current therapy was pimobendan.

DX

Mild DMVD causing very mild mitral insufficiency.

Sonographic Differential Diagnosis

This is a case of mild DMVD causing very mild mitral insufficiency. The left atrium is only very mildly increased, the left ventricle doesnВґt show any volume overload. I cannot see CHF in this case. I would stop with Pimobendan and recheck the patient after one year.

Image Interpretation

Normal (no volume overload) left ventricle, a very slightly enlarged left atrium and a mildly degenerated mitral valve. The posterior leaflet is more affected than the anterior and is very slightly prolapsing into the LA. There is very mild to mitral insufficiency present. The LVOT including CDI and spectral flow is normal, no sign of turbulence or stenosis. The right ventricle and atrium are normal in terms of size, the tricuspid valve doesnВґt show pathologic changes. Flow across the RVOT is normal, there is neither a stenosis, nor pulmonary hypertension present. This dog seems to have a quadricuspid aortic valve, a very rare finding (without hemodynamic sequelae. No pericardial effusion, no tumor visible.

Clinical Differential Diagnosis

Stable heart disease, progressive mitral/tricuspid endocardiosis, myocardial weakness
