Thoracic mass suggestive for lung neoplasia in a 6 year old MI Rhodesian Ridgeback dog

A 6 year-old male Rhodesian Ridgeback with a history of IMHA and current cortisone therapy was presented for being depressed. Abnormalities on physical examination were pale pink mucosa, tachycardia, polypnea, and a distended abdomen. CBC showed leukocytosis, neutrophilia, monocytosis, and regenerative anemia. Coombs test was negative.

DX

Right thoracic mass suggestive for lung neoplasia

Sonographic Differential Diagnosis

Right thoracic mass suggestive for lung neoplasia, sarcoma suspected. Other potentials include lung lobe torsion or pneumonia but these are less likely given the case history and complete lack of architecture in the consolidated tissue.

Image Interpretation

The near field corresponding to the right thorax reveals a mixed hypoechoic mass impinging upon the right heart. Cardiac chamber size and contractility are normal.

Outcome

The patient was euthanized 10 days after the ultrasound exam.

Clinical Differential Diagnosis

Cortisone side effects – Cushing’s disease, hepatopathy, gastric ulceration, secondary bacterial infections, Addison’s disease. Ongoing IMHA Splenic disease – hypersplenism, infarction, torsion, neoplasia, infectious Cardiac – cardiomyopathy, pericardial effusion

Sampling

None permitted by owner. US-guided FNA or core bx would have been appropriate.
