A 3-year-old neutered male Golden Retriever dog, with history of seizures and sub-aortic stenosis, presented for a re-evaluation to determine if the heart defect was stable and if any interventions would be appropriate at this time.

Sonographic Differential Diagnosis

Compensated subaortic stenosis with secondary aortic insufficiency and mitral insufficiency. Aortic and mitral insufficiency have progressed compared to prior sonogram.

Image Interpretation

The cardiac images demonstrated normal atrial size with minor mitral valve thickening with adequate apposition. However, mitral insufficiency was prominent, and considered moderate at 5.6 m/sec. Unfortunately, the mitral insufficiency was not noted on prior sonogram, which is consistent with a minor degenerative setback. The left ventricular internal volume, contractility, and septal free wall thicknesses were all normal. Aortic outflow did demonstrate valvular thickening and a subaortic lembus (see comments) deriving from the ventricular septum. This lembus was consistent with what is often found with subaortic stenosis. Aortic outflow velocity was mild to moderately elevated at 3.8 m/sec. However, aortic insufficiency was significantly increased compared to the prior sonogram and was moderate at 5.2 m/sec.

Outcome

No treatment was recommended at this time. The lesion appears compensated but progressive.
