Lymphoma in a 4 year old FS DSH cat

A 4-year-old FS DSH cat was presented for intermittent vomiting over the course of the last few months. Recent blood work was unremarkable. The only significant finding on abdominal radiographs was a full colon.

DX

Intestinal lymphoma, high grade, duodenum

Sonographic Differential Diagnosis

Inflammatory or neoplastic gastrointestinal disease.

Image Interpretation

Image 1: Proximal duodenal thickening immediately distal to pylorus Video 1: Duodenal thickening distal to pylorus showing loss of mural detail and ill-defined, somewhat hyperechoic, possibly reactive mesentery in the associated tissue.

Outcome

The owner consulted with an oncologist but declined chemotherapy. The pet was started on prednisone, Pepcid and Carafate. Six months later the pet was re-evaluated for anorexia and lethargy and at which time was euthanized.

Comments

This duodenal lesion was very subtle and emphasizes the need to focus on loss of mural detail as well as the need to image the entire organ system. This small lesion was responsible for the clinical presentation in this pet. Such a lesion could have easily been missed owing to gastric or colonic artifact. The lesion was also identifiable by means of recognizing the + Murphy sign presented by the patient when imaging this region indicating perceived pain.
