An 11-year-old MN Golden Retriever was asymptomatic and presented for a geriatric workup. Physical examination was normal. Laboratory analysis revealed moderately elevated SAP, moderately elevated cholesterol, and moderately elevated GGTP. Urinalysis was normal.

Sonographic Differential Diagnosis

Chronic inflammatory hepatopathy with nodular hyperplasia and neoplasia.

Image Interpretation

Transverse image of the left liver reveals diffuse hyperechoic, heteroechoic parenchyma and a solitary, discrete 1.3-cm hypoechoic nodule.

DX

Biopsy parenchyma: vacuolar hepatopathy. Biopsy nodule: benign nodular hyperplasia

Outcome

The patient remained asymptomatic over the next 6 months with gradual low-grade increased SAP values.

Clinical Differential Diagnosis

Liver pathology: low-grade hepatitis, vacuolar hepatopathy, biliary disease, neoplasia.

Sampling

16-gauge US-guided biopsy of the random parenchyma revealed vacuolar hepatopathy, whereas biopsy of the solitary hypoechoic nodule demonstrated benign nodular hyperplasia
