What is this I’m looking at?

This is my assistant’s dog that we were just imaging to try out a new machine and found this on the vental aspect of the Urinary bladder near the neck. It did not elongate with transducer manipulation and no vascularity seen. Clinically normal. Lymph node?

It looks like a lymph node. I dont see evidence of inflammation around it and the subjective width: length ratio looks within normal limits, yet its slightly hypoechoic and rounded. 

I dont see evidence of UB or proximal urethra issues so it may just be incidental. I would watch it with FNA if it persists or changes in size.

Is that a common area to find a lymph node? I’ve never seen one there before.

Yes I agree its an abnormal area for a lymph node. You would expect a lymph node to be dorsal to the UB, unless its a deep inguinal lymph node but it appears to be to close to the UB for that. So its odd…

As I look at it closer, I believe its associated with the UB wall actually. You can see the UB wall actually go around the lesion. It looks solid to me a cyst is possible. You can try to put Color Doppler on it to see if you can get any vascularity associated with it. 

I would recommend FNA of it now that I look closer at it.

I know this may sound

I know this may sound rediculus- but when I look at the cine it appears that the orientation of the bladder may be reversed from what we normally see. If that is the case could this be a abnormally large and compartmentalized persistant urachus? Just a thought.

She did and I did aspirate

She did and I did aspirate the lesion but unfortunaley it was non-diagnostic despite feeling confident I was in the lesion. The dog is completely asymptomatic at this point so we’re going to follow it for a bit and decide whether to re-aspirate going forward.
