Feline incontinence and thickened urethra

Babs DSH, 4 y, FS, 7# kitty has been having urinary incontinence since May 2018. She has had a urine culture which is negative.
Her owner reports she voids in the litter box and there are about normal size clumps of urine. She does not report vocalization or stranguria while voiding. She has hematuria and drips urine frequently and not associated with sleeping.
CBC, wellness chemistry were normal and FELV/FIV testing negative. Owner was not sure exactly when pet had been spayed.

My concern is the appearance of the urethra. It appears significantly thickened. The patient was aggressive so was sedated for the exam using a sedative protocol TTDex. (telazol, torbutrol, dexmetomidine). She did leak hemorrhagic urine during the exam. If she were a dog I would be thinking TCC of urethra.
Palpation of the urethra rectally did feel thickened to me however I can’t recall very many feline urethra’s that I have palpated but it felt similar to some dogs with thickened urethra that I have palpated.

That is a thick urethra and would be significantly abnormal to be Tcc here in  4 YEAR OLD CAT.

maybe free catch a urine and cytospin it and slide out sedminent?
