Dry FIP ?

The right kidney is small (2.8 cm length) with hyperechoic irregular cortex.  The left kidney is 4.4 cm, with much more normal architexture.  There is one possible cortical lesion on the left kidney?

No other abnormalities in the abdomen.

Is there any way to differentiate between CRF from unknown causes and dry FIP?

These are chronic

These are chronic interstitial nephrosis patterns but they have a medullary rim sign whihc can be idiopathic or related to FIP. Would need bx to know> With the clinical history and this pattern its suspicious especially with the pathcy cortical changes most evident in the first video. Good kidney to Bx as these are usually fibrotic type kidneys and less likely to have complications with us guidance looking for granulomatous changes. Also look for similar patchy changes in the liver, any lymphadenopathy or chronic ropey bnowel. I have found that a numnber of those chronic triad looking older cats that have a tough time responding to tx are actually dry fip cats if you get solid bx (usually surgical ones) on them.
