- Not up to date with vaccinations
- Severe azotaemia on bloods, urine sg 1.020, protein 3+ (dipstick)
- Very painful on ultrasound of kidneys and difficult to get good images
- Full urinalysis and lepto PCR pending
- How would you describe these kidneys and what would your differentials be please?

Hyperechoic, irregular
Hyperechoic, irregular capsule, loss of cortico-medullary differentiation, pylectasia: end stage renal disease. Possible etiologies – congenital renal disease, prior toxins/hypovolumia, Leptospira, Lyme, bacterial nephritis. Biopsy at this stage often non-diagnostic. Woud culture urine as there may be a secondary UTI which is complicating the picture. Manage as for chronic kidney disease – renal diet, phosphate binders, gastric protectants, EPO as needed.
