This is a 4 years old F/S that has “chronic innapropiate urination”
UA : very cloudy urine USG 1.030, Hematuria with 10-20 / HPF, WBC 20-30 / HPF, Triple phosphate 10-50/ HPF
Not responsive to antibiotics (clavamox).
Bladder, trigone normal with no urolith or stone, kidneys no pylectasia.

It looks like a straight forward case of UTI ( culture is pending). My question is what does make the Hypoechoic cortex? I know that sometimes lymphoma or any acute disease can cause edema, but this looks like a chronic case. Should I be concerned about Kidneys appearance, or am I overread..?

This is a medullary rim sign
This is a medullary rim sign which is idiopathic. May be owing to tubular disease and I make more of a deal of it when there is proteinuria but often its an nsf thing.

If urine culture is negative
If urine culture is negative then interstitial cystitis would be an important diagnosis
