– 9 yo MN DSH, weight loss over the last few months, vomiting (hairballs, sometimes food) 2/week, intermittent decreased appetite.
– Hx of UO 2 years ago, fed Urinary SO 2 cans BID. Hx heart murmur, normal echo 2 years ago.
– Bloodwork done when weight loss first noted 4 months ago, all WNL.
– I scanned him at home (not NPO, standing with my daughter holding him).

– Diffuse splenomegaly (1.6cm), benign vs lymphoma vs MCT, mild thickened intestines (2.8mm) with a prominent muscularis on half of the loops, prominent hyopechoic colic and jejunal LN, up to 8.5mm wide.
– Differentials: IBD vs lymphoma vs dry FIP.
– Plan: repeat labwork, FNA LN and spleen this week at the clinic.
– I know we can’t tell by just looking at the LN, but what is your clinical impression?

Update: PARR positive. Intestinal T cell lymphoma. I knew it was serious when Eric said he didn’t like that spleen. Thank you for guiding me, it’s always tougher on our own pets.
Diagnosis
Spleen: Small lymphocyte proliferation
Lymph node: Small lymphocyte proliferation
CSU PARR Results:
Immunoglobulin gene: Polyclonal T cell receptor gene: Clonal
The PARR assay revealed a clonally rearranged T cell receptor gene. This finding is supportive of T cell neoplasia. The specificity of a positive result with the PARR assay has been determined on blood and lymph nodes from cats with no clinical, cytologic or histologic evidence of lymphoma (n = 21 samples). No false positive results were detected in these samples (Rout, 2019 Vet Clin Path 48 S1:45-58). The specificity of the assay when used on non-lymphoid tissue has not yet been determined. The specificity of the assay in cases where lymphoma is suspected but not confirmed has also not been determined.
Intestinal T cell lymphoma in cats is most commonly a small cell indolent form. However a minority of cases have a more aggressive clinical course.

I researched Small Cell Lymphoma treatment for my own cat (who’s case didn’t pull together so neatly) and its very doable with a decent prognosis. Yay for your cat!

So sorry to hear it didn’t go well on your own cat… He’s been on pred for a few days, starting chlorambucil 2mg tomorrow, once I get Cerenia on board. I hope he will read the book and be a regular small cell lymphoma cat.
