Ultrasound of the right shoulder –
The infraspinatus, deltoid and part of the triceps muscles present a marked and irregular increase in echogenicity with volume increase and partial loss of the regular echoarchitecture. The muscles bellies are contained within their fascia. There is no fluid accumulation within or between the muscles. The visible bone surfaces of the scapulary spine and fossa infraspinata are even and smooth.
The findings are not typical for septic myositis or full muscle rupture. For both entities the potential of fibrotic replacement of the muscle fibres with development of contracture has to be considered a possible long term complication. Prevention of fibroplasia and early mobilization is crucial and may be supported by pain relief and muscular reconditioning (deep heat ultrasound or laser, massages, unforceful stretching). Methocarbamol, a centrally acting muscle relaxant, may be used in a clinical trial in order to decrease muscular spasticity and possible pain.
Injection of platelet-enriched plasma may be considered as well.
Surgical treatment should be considered only in case the patient reveals persistent locomotory deficits.
