A 7-year-old SF beagle was presented for evaluation of PU/PD. Urine specific gravity was 1.010 and serum biochemistry showed elevated ALP (1720) activity and cholesterol (413).

 

Sonographic Differential Diagnosis

Left adrenal mass. Given the patient’s history there is a strong suspicion for adenocarcinoma. This appears resectable. Differentials include adenocarcinoma, possible pheochromocytoma, which can mimic Cushingoid parameters at times.

Image Interpretation

The left adrenal gland comprised a mixed echogenic expansive mass with pericapsular inflammatory pattern. The left adrenal mass measured 2.88 x 2.5 cm deriving from the caudal pole and the cranial pole measured 0.62 cm. The phrenic vein was clear and there was no evidence of invasion or capsular escape. The right adrenal gland was flattened and isoechoic. This is likely owing to positive feedback atrophy.
